Wanderoff is the first hospital in World 9 of Two Point Hospital, which requires the Off The Grid DLC. To unlock it, the player will need to reach Star Level 
in Flemington. It is recommended, however, that the player progress at least as far as Grockle Bay and Sweaty Palms, so as to unlock several treatment rooms, most notably the multi-illness rooms Surgery and DNA Lab, that will be required in the DLC. Upon reaching Star Level in Duckworth-upon-Bilge, Wanderoff will automatically be unlocked, whereas it would previously have had to be unlocked manually.
The Off The Grid hospitals will all play host to a new VIP Visitor, Flanolin Onionsack, a representative of Two Point County's Department of Green Things. A new playstyle, Index Challenge Mode makes its debut in Wanderoff; with income from the usual sources greatly reduced, instead the player's expenses will be covered by the Holistix corporation, based on the hospital's performance. A new clinic illness, Woodworms, will make its first appearance, along with the Woodwork room, in which it can be treated.

It's that time again. Our County is approaching another mayoral election, and I've decided that beginning our journey towards a greener tomorrow is long overdue. Our voter research suggests that it's the issue on everyone's mind, and I am willing to fall on the sword of extensive positive press by founding the Department of Green Things.
When people remember Tabitha Windsock, I want them to remember my green initiative and how I got re-elected that time. Please, support the initiative by taking care of our green-minded, recently-pro-windsock, community. Welcome to Wanderoff County Park, where Holistix have been farming rare, natural ingredients for years. Once they heard that the Mayor was planning to trial her green initiative in the area, they decided to open a nature commune, and take advantage of the foot traffic. It's our job to care for the people attached to those feet. Our funding from Holistix will depend on how well we're scoring on their new scale...
Yes, the (less than) fine people at Holistix have developed something called a "Wellness Index", which uses a number of different criteria to measure the overall wellbeing of a community. They believe that they've found a correlation between high scores on the Wellness Index and a willingness to purchase their organic tat. We won't receive much for diagnosing and treating patients, so if we want to make money we'll need to score well on the Wellness Index by focusing on its six criteria: staff and patient happiness, attractiveness, hygiene; reputation; and cure rate.

As part of the Mayor's green initiative, your hospital will receive visits from the Department of Green Things and be given Eco Ratings.
Eco Ratings may result in bonuses, or fines, from the Mayor, depending on whether you're helping, or hindering, her re-election campaign.
Wellness Index[]
Wanderoff features an expanded version of Underlook Hotel's Index Challenge Mode. Every three months the hospital will be assessed by Holistix. At this time, the proportion of the hospital's running costs covered for the next three months will be determined. This does not include the cost of placing rooms and items, trainer fees, or hiring fees for new staff members.
Between 50% and 120% of the hospital's staff wages and energy bill will be covered, meaning that in a high-performing hospital, it is possible to make money by hiring staff. Unlike in Underlook Hotel, where only the hospital's cure rate was being taken into account, here the amount covered is determined based on performance in that, as well as five additional categories:
