Patients[]
- Main article: Patients
The majority of visitors to the hospitals will be standard patients. They will need to undergo a full course of Diagnosis and Treatment before being discharged - if they survive!
Emergency and VIP Patients[]
- Main article: Emergencies
A special type of patient will be heralded by a letter in the player's inbox. There are two types of such emergencies.
Mostly, these will consist of a set number of patients with a particular illness, all of whom will arrive upon the player accepting the case (although refusal is an option if the player does not have the means to cope with it). They will be fully diagnosed upon arrival, and as such, will proceed straight to the appropriate treatment room.
In Lower Bullocks and Pebberley Reef, another type of emergency will occur. In this case, the letter will offer the player a single VIP patient, who will require the full diagnosis before treatment.
In both cases, the patients in question will have a blue-and-red flashing emergency light above their head, and have their name suffixed with "(VIP)". Each emergency will have a set of rewards based upon the number of successful treatments.
VIP Visitors[]
Each hospital has a roster of VIP Visitors that may visit in order to conduct an inspection. A letter will arrive in the player's inbox, announcing their intentions. Unlike Staff Objectives or Emergencies, which can be safely filed away for later, VIPs will not wait, and will take silence as consent. In some cases, the visit may be refused; others, such as Henry Jobsworth, will arrive whether the player is ready or not.
Marked by the three-pointed crown icon above their head, the VIPs will stroll around the hospital, visiting between three and six rooms, while also inspecting the Corridors and talking to Staff and Patients along the way. Each VIP will have their own areas of focus, such as the environment itself, patient care, staff wellbeing and so on. They will also have a set of rooms they prefer to visit, if available. These are all specified on each VIP's page.
Having judged the hospital by their preferred criteria, the VIP will then leave the hospital, upon which the player will receive another letter, this time with the results of the assessment. If they are pleased, the player will receive rewards. If not, they will instead reduce the hospital's reputation, or occasionally impose a monetary fine.

Gretchen Gearbox[]
Gretchen Gearbox

The player's newest ally, Gretchen Gearbox is tasked with creating, upgrading and maintaining the ambulance fleet, as well as passing on her skills to Janitors through Staff Training.
Guest Trainers[]
- Main article: Staff Training
Guest Trainers can be deployed in order to teach new Qualifications that are not possessed by an existing member of staff, or simply if such a member of staff is needed elsewhere. For a fixed cost per session, plus an additional fee per student, they can be summoned to a Training room in order to conduct a lesson.
They are fixed characters, as opposed to hirable staff, and as such can only teach one course at a time. Between them, they cover all of the skills that exist in the game, with the exception of Vehicular Mechanics, which must be taught by Gretchen Gearbox.
