Spooooooky Mode is the annual seasonal update that sees Halloween come to the hospitals of Two Point County.
A cosmetic update, Spooooooky Mode sees several hospitals appear at night rather than day, several items and environments featuring Halloween-themed decorations, new music tracks and even Lightheadedness being temporarily replaced by Frightheadedness.
First released on 23rd October 2018 running, until 5th November 2018, Spooooooky Mode would make return appearances on 21st October 2019, 27th October 2020 and 28th October 2021, each lasting a few days.

Shortly after, on the 26th October, Sandbox: Freeplay update was released. They where advertised together with a 10% off discount in the 2018 Steam, Halloween Sale.
Features[]
Many cosmetic changes where made. Each have been listed below with images in the gallery.
- Night Hospitals - During the 2018 event, Mitton University and Tumble were changed from day time to night time hospitals. Inside, the buildings remains brightly lit, but the outside is dark. The street lamps remain off but the car lights and ambulance lights flash brightly in the dark. In 2019, instead Hogsport, Melt Downs, Roquefort Castle and Goldpan receive the night-time treatment. In these cases, the street lamps are active.
- Hospital Decorations - All hospitals have new spooky decorations at every entrance. Main entrances have a collection of pumpkins, two of which have glowing faces. All other external doorways have pumpkins but also a big black cauldron with green liquid steaming inside. Bats also fly across all hospitals. They can be seen alone or in groups.
- Music - Six original tracks are replaced with new spooky versions. All these tracks, with a sample, are listed below in Music. They are also listed on the Two Point Hospital page.
Cosmetic Item Changes (2018)[]
- Reception (Item) - Pumpkins can be found behind the desk with one sitting proud on top.
- Anatomy Model - A Jack-o'-lantern head is added.
- Dog Painting - Green goo cries rivers from it's eyes.
- Office Desk - The lamp is removed to make room for a big Jack-o'-lantern.
- Display Skeleton - Gains a Jack-o'-lantern head.
- Newsagent - The 3 teddy bears are placed with 3 pumpkins and the newspapers on the opposite side are replaced with 3 Jack-o'-lanterns and a small cauldron filled with the green goo.
- Gift Shop - The gift boxes are replaced with 1 big pumpkin and a small cauldron filled with the green goo.
- Drawing Board - A large Jack-o'-lantern sits on the stool facing the drawing board.
Cosmetic Item Changes (2019)[]
- Reception Desk - A spider's web appears on the main surface, with another below the desk on the left hand side, as well as a skeletal hand emerging from the pencil holder.
- Arcade Machine - A spider's web stretches across the screen.
- Ward Bed - A spider's web appears in one corner, as well as a skeletal hand emerging from under the bed.
- Brain Jar - The brain in the jar has been replaced by a pumpkin.
- Carnivorous Plant - The plant is wearing a pumpkin mask.
- Cat-in-the-Box - The box is orange and the cat is black.
- Display Skeleton - The skeleton's head has once again been replaced by a pumpkin, although in a slightly different orientation to that of the previous year.
- Dog Art - Green goo cries rivers from it's eyes.
- Dog Canvas - Tears flow from it's eyes.
- Dog Painting - Green goo cries rivers from it's eyes. Also appeared last year.
- Gnome - Wearing a pumpkin mask.
- Mountains Painting - The water spills out of the frame.
- Newsagent - A spider's web stretches vertically down the left-hand side of the display.
- Pink Bird - Wearing a pumpkin mask.
- Toilet Cubicle - A spider's web stretches across the top of the cubicle.
Cosmetic Item Changes (2020-21)[]
- Reception Desk - A jack-o-lantern sits on the left-hand side of the desk, and spiders' webs can be seen on the desk surface on the left, and underneath on the right.
- Newsagent - A jack-o-lantern sits on the right-hand side, with another on the left-side shelf, and a third replacing the head of a teddy bear. A spider's web is seen on the left.
- Nurse Station - A disembodied skeletal arm is found in the bedpan.
- Ward Bed & Jasmine Bed - A skeleton, relaxing with arms crossed behind its skull, appears under the bed
- Anatomy Model - A jack-o-lantern is placed atop the neck
- Display Skeleton - A jack-o-lantern replaces the skull
- Brain Jar - A jack-o-lantern replaces the brain itself
- Cat-in-the-Box - The box itself is now orange, while the cat inside is black
- Dog Art - Crying green tears
- Dog Painting - Crying tears
Frightheadedness[]
Yes, Spooooooky Mode even gets its very own illness. The illness Lightheadedness is morphed into the beautiful but equally alarming Frightheadedness. Instead of having lightbulbs for heads, these more folk have Jack-o'-lanterns for heads.
The cure is still a trip to the De-Lux Clinic, but instead of unscrewing the bulb, the machine grabs the pumpkin head and pulls, until the pumpkin is plucked from the stalk neck. The flame inside the Jack-o'-lantern extinguishes and is thrown away as a new head is popped back onto their shoulders.
